registerNew Patient Registration

Anyone wishing to register with the practice should bring along two current utility bills or a bank statement, tenancy/mortgage agreement, council tax bill and passport.  We will ask you to complete a form which will enable us to collect all other relevant information.  When completing the form you may indicate your preferred doctor but this does not guarantee that you will always see the same doctor. 

You are registered with the practice team as a whole.  Providing you are within the catchment area and have attended your new patient health check your registration should be completed within 24 hours.  The practice area covers both Hainault and Chigwell and some parts of Barkingside.  Please ask at reception for clarification.  

You can register any morning from 09:30 - 13:00 except for Thursday when you can register between 15:00 - 18:00. You can collect the registration forms at any time. 

We would also encourage you to register with 'The Waiting Room' which is our on line service.  This will enable you to book appointments and order your medication. 50% of our appointments are available to book in this way which will save time and effort contacting the surgery by telephone.  Please see the receptionist for further information and an application form.

Change of Address

It is essential that we have your up-to-date contact details; please inform us immediately of any change of address or contact telephone number (including mobile number). Please provide us with your email address.  We need to see official paperwork for any changes.  For change of name due to marriage we would need to see your marriage certificate.  For change of name by deed pole we need to see the official paperwork and any address change we would need to see a current utility bill etc.
