Patients should self-certify for the first six days of illness using form SC1 obtained from the employer.  For illness lasting longer than six days or expected to last longer than six days you will need to obtain a sick certificate from your doctor.

(For certification the patient must be seen by the issuing doctor, or this issuing doctor must have seen a recent written report from the doctor who has examined the patient).  NHS sick notes attract no fee.  For periods of less than six days or for retrospective sick notes without an accompanying medical report, a private sick note can be issued at a fee of £18.  Other non NHS sick notes are also chargeable.  Please ask at reception for details.

There is no charge for parking badge assessments, NHS/DSS work for DVLAS etc.
